To improve access, safety, and efficiency, we have moved to a digital-first model of care.
📋 All Appointment Requests Must Be Submitted Online
All requests for appointments, medical advice, or admin queries (including repeat prescriptions, sick notes) must now be submitted using our online triage form, available on our website.
This system ensures your request is directed to the right clinician or service quickly and safely.
Appointments for the Practice Nurse, Nurse Associate or Health Care Assistant can still be booked by contacting the surgery.

Once submitted, your triage form will be reviewed by our clinical team.

You will receive a response within 48 working hours — this may be a call, message, prescription, or appointment depending on your need.
💬 Need Help?
If you are unable to use the online form yourself, a family member, friend, or carer may complete it on your behalf. Alternatively, our reception team can help you fill it in if needed.
